What your day could look like

As Coordinator Home Care, you'll help older people remain independent and supported at home. Working with clients, families and service providers, you'll coordinate quality care and lead a team focused on positive outcomes across the community.

Every day, you'll contribute by:

- Coordinating personalised support that empowers clients to achieve their goals and make informed choices
- Building strong partnerships with carers, health professionals and community services to deliver seamless care
- Leading and supporting home care staff to maintain safe, responsive and high-quality services
- Driving service improvements, compliance and resource management that strengthen client outcomes

You're someone who combines compassion with sound judgement and enjoys helping people navigate important life stages with confidence. You build trusted relationships, adapt to changing needs and take pride in creating positive experiences for older people and their families. The difference you make is seeing clients supported to live well at home while receiving care that reflects their goals, dignity and independence.

Here's what will help you thrive in this role:

- Experience coordinating community-based or aged care services, including care planning and case management
- Strong communication skills and the ability to work collaboratively with clients, carers and multidisciplinary teams
- Experience leading staff and managing priorities in a dynamic service environment
- Confidence using digital systems, databases and administrative processes to support quality care





You'll stand out from the crowd if you have a Certificate III or higher in Individual Support (Ageing) or similar and/or experience working with aged care quality standards.

Above all, you'll bring compassion, integrity, dignity, innovation and courage to every interaction, helping deliver care that matters.
