- Location – Office based at Hunters Hill and visiting clients throughout the Northern Sydney area
n
- Permanent Full-Time role (Monday to Friday)
n
- $48.00 per hour+ superannuation + Km reimbursement( $1.11 per km)
n
- Up to $18,550 tax free with salary packaging

In the challenging and rewarding role of Care Partner, you will be providing relationship-centred care management that delivers exceptionalclientoutcomes,strengthens wellbeing, and supports clients to thriveand livesafely at home.

Day to day this role looks like:

n

n
- Achieving billable hours and service delivering expectations through effective care planning, funding utilisation, billing accuracy, and proactive management of client portfolio
n
- Undertaking person-centredcare planningin accordance withCatholic Healthcare'smodel of care
n
- Educating clients and their families on program guidelines, inclusions and exclusions and possible financial contributions
n
- Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
n
- Using screening,assessmentand risk dashboards toidentifyhigh riskclients and prioritizetimelyreviews
n
- Initiating case conferencing to support hospital, rehab or respite discharge planning forwellbeing interventions, serviceadjustmentsandtimelyreferrals
n
- Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs,



planinterventions, and review outcomes of care
n
- Promoting Catholic Healthcare Value proposition and service offerings while enabling clients to live at home independently with choice, control and connecting them to services that are secure,personaland effective
n
- Ensuringaccuratebilling,timelydocumentationand alignment of services with client goals and funding availability
n
- Understanding and adhering to the Aged Care Standards in all interactions,practicesand documentation
n

To be successful in this roleyou'll need:

n

n
- Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards
n
- Strong understanding of the ageing process, particularly age-related health issues
n
- Case management experienceincludingassessment, care planning, goal writing and reviewing
n
- Previousexperience working within acommunity settingin a similar role
n
- Ability to work independently and as part of a multi-disciplinary team
n
- Excellent digitalliteracy, including Microsoft Outlook and Excel
n
