Care Partner – Support at Home

Work location: In person – travel between client locations required
Caura is seeking a warm, experienced Care Partner to build genuine, ongoing relationships with our Support at Home clients. This role is the trusted human face of care — getting to know each client and their family, understanding their goals and changing needs, and making sure their voice stays at the centre of every decision.
We're looking for someone who sees care as a partnership, not a checklist — someone who can sit with a client, listen, advocate on their behalf, and help them get the most out of their Support at Home services. Previous experience in the aged care sector is highly valued.
Key Responsibilities
- Build and maintain a trusted, ongoing relationship with a caseload of Support at Home clients and their families
- Conduct regular in-home visits and check-ins to understand each client's changing needs, goals, and wellbeing
- Develop and review individualised care/support plans in partnership with clients, families, and support workers
- Act as the client's primary advocate, ensuring their choice and control are central to every decision
- Identify changes in health, wellbeing, or risk, and respond promptly, escalating where needed
- Help clients and families understand their Support at Home budget and the services available to them
- Work alongside the service coordination team to make sure the right support is in place
- Provide reassurance and emotional support to clients and families navigating the aged care system
- Respond to and resolve client or family concerns with empathy and professionalism
- Maintain accurate case notes reflecting client goals, conversations, and outcomes
Skills & Attributes We're Looking For
- Previous experience in the aged care sector, ideally in a client-facing case management or care partner role
- Warm, empathetic communication style with strong relationship-building skills
- A person-centred, trauma-informed approach to care
- Cultural awareness and respect for diversity
- Ability to listen, advocate, and problem-solve on behalf of clients
- Comfortable working independently in clients' homes and communities
- Understanding of the Support at Home program and/or the Commonwealth Home Support Programme (CHSP)
Essential Requirements
- Previous experience in the aged care sector is preferred (essential for this role)
- Relevant qualification in aged care, community services, social work, nursing, or a related field (Certificate III/IV, Diploma, or higher highly regarded)
- Working knowledge of the Support at Home program and Aged Care Quality Standards
- Genuine passion for supporting older Australians to live independently and with dignity
Mandatory Requirements
- Valid driver's licence and access to a reliable vehicle
- Police Check (or willingness to obtain one)
- NDIS Worker Screening Check (or willingness to obtain one)
- Working With Vulnerable People / Working With Children Check (or willingness to obtain one)
- First Aid and CPR certification (or willingness to complete training)
- Full working rights in Australia
- Government-issued ID for verification purposes
