Multicultural Care is a leading not-for-profit organisation providing culturally sensitive home and community-based support to older people, people with disabilities, and their carers. We are seeking an experienced and values-driven Care Partner who is responsible for the provisioning of high-quality case management, support planning, and care coordination services across all home care program types (including SAH and CHSP).
Key Responsibility Areas:
· Deliver Home Care Services
Ensure all programs comply with internal policies, legislation, standards, OH&S; requirements and risk management procedures.
· Person-Centred Assessment & Care Planning
Conduct comprehensive and initial assessments, determine priority of need, develop client-centred care plans and consumer budgets, and complete mandatory reviews.
· Case Management & Client Support
Provide information, advice and referrals to support client/carer choice, autonomy and access to appropriate services while protecting privacy and rights.
- Care Coordination & Escalation
Coordinate services, manage changes in client circumstances, escalate urgent issues, and consult with the Team Leader when required.
· Data, Documentation & Reporting
Maintain accurate and timely records, ensure high-quality data entry into client management systems, and meet payroll, finance and reporting deadlines.
· Compliance & Risk Oversight
Monitor adherence to policies and guidelines, identify and manage risks, and document incidents according to organisational procedures.
· Professional Practice & Development
Apply ethical decision-making, work within qualified scope, participate in performance reviews, maintain professional knowledge, and engage in continuous improvement.
· Clinical Responsibilities
Provide clinical assessments and care planning for complex clients, offer clinical guidance to non-clinical staff, conduct clinical risk assessments, and contribute to best-practice clinical frameworks.
Essential:
· Tertiary qualification and experience in Allied Health, Nursing, Social Work, or a related field and relevant experience in case management evidenced.
· Good computer & verbal skills, with competence in MS Office and Client Management applications.
· Experience in assessment and development of care plans & setting and management of consumer budgets
· Experience in provision of case management to consumers with complex needs
· Understanding of Consumer Directed Care principles and Aged Care Reforms.
· Familiarity with community aged care services and referral pathways
· Demonstrated commitment to high quality customer service & Sound networking skills along with needs and issues of older people from CALD background and their carers;
· Ability to work autonomously and in team as well; sound time management skills and ability to prioritise to meet competing demands
· Valid NSW Drivers licence and access to own vehicle
