Clinical Care Coordinator – HomecarePosition Overview The Clinical Care Coordinator – Homecare is responsible for providing high-quality clinical oversight, care coordination, assessment, and support to clients receiving services in their homes and community settings.

The role ensures that clients receive safe, person-centred, culturally respectful, and clinically appropriate care while supporting care staff and coordinating effectively with families, healthcare professionals, allied health providers, and other stakeholders.

The Clinical Care Coordinator will contribute to clinical governance, risk management, quality improvement, and compliance with relevant legislation, professional standards, organisational policies, and funding requirements, including Support at Home and NDIS programs where applicable.

Key ResponsibilitiesClient Assessments

- Conduct comprehensive clinical and functional assessments using appropriate standardised assessment tools.
- Identify clients’ individual care, health, functional, social, and support needs within the home environment.
- Regularly reassess clients in response to changes in health, function, cognition, behaviour, or circumstances.
- Identify risks and opportunities to promote independence, wellbeing, and quality of life.

Care Planning
- Develop, implement, monitor, and review individualised care and support plans.
- Ensure care plans reflect current clinical needs, client preferences, goals, risks, and support requirements.
- Ensure care plans comply with clinical best practice, quality standards, funding requirements, and relevant programs, including Support at Home and NDIS, where applicable.
- Communicate care-plan changes clearly to clients, families, care workers, case managers, and other relevant team members.

Clinical Leadership
- Provide clinical guidance and advice to community care workers, case managers, rostering staff, and other members of the care team.
- Provide supervision, coaching, education, and competency-based training within the scope of practice.
- Support staff to understand and appropriately implement clinical recommendations and care plans.
- Promote evidence-based, safe, and person-centred care practices.

Stakeholder Communication
- Act as a central point of clinical coordination between clients, families, carers, GPs, nurses, allied health professionals, hospitals, and other service providers.
- Establish and maintain effective professional relationships with internal and external stakeholders.
- Facilitate timely communication regarding changes in client needs, risks, incidents, referrals, and care requirements.

Care Coordination
- Coordinate the delivery of safe, appropriate, and person-centred services.
- Ensure services are delivered in accordance with approved care plans and client goals.
- Liaise with relevant teams to coordinate changes to services, clinical interventions, referrals, and supports.




- Monitor service delivery and address identified gaps or concerns.

Clinical Monitoring and Escalation
- Monitor clients for changes in health, function, cognition, behaviour, and overall wellbeing.
- Identify signs of deterioration or emerging clinical risks and initiate appropriate escalation.
- Coordinate referrals to GPs, nurses, allied health professionals, hospitals, or other relevant services when required.
- Provide appropriate clinical interventions within professional scope of practice.
- Ensure urgent or significant clinical concerns are escalated promptly and appropriately.

Risk Management
- Identify, assess, document, and manage clinical, environmental, and psychosocial risks.
- Monitor and manage risks including:
- Falls and mobility concerns
- Medication-related concerns
- Pressure injuries and skin integrity
- Functional decline
- Malnutrition and hydration concerns
- Changes in cognition or behaviour
- Infection risks
- Environmental and home safety risks
- Safeguarding and abuse concerns
- Develop appropriate risk-management strategies and review their effectiveness.

Documentation and Compliance
- Maintain accurate, timely, objective, and confidential clinical documentation.
- Ensure records comply with organisational policies, qualified standards, privacy requirements, and relevant legislation.
- Maintain appropriate documentation of assessments, care plans, clinical reviews, incidents, referrals, and clinical communications.
- Support compliance with relevant aged-care, disability, health, and quality standards.

Quality Improvement and Clinical Governance
- Participate in clinical audits, incident reviews, case reviews, and quality-improvement activities.
- Identify opportunities to improve clinical outcomes, client experience, safety, and service delivery.
- Contribute to the development and review of clinical policies, procedures, and care practices.
- Promote a culture of continuous improvement, accountability, and evidence-based practice.

Staff Support
- Support care staff with clinical escalation and interpretation of care plans.
- Provide guidance regarding appropriate responses to changes in clients’ health or functional status.
- Identify staff education and competency needs and support appropriate training and development.
- Promote safe work practices and appropriate professional boundaries.

Qualifications and Mandatory Requirements
- Bachelor of Nursing or equivalent recognised nursing qualification.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Current First Aid and CPR certification.




- Current Police Check/Police Clearance as required.
- Current NDIS Worker Screening Check and/or Working with Children Check, where applicable to the role and client cohort.
- Current driver’s licence and reliable access to a suitable vehicle, where required for community-based work.
- Commitment to maintaining mandatory training and career development requirements.

Experience
- Experience in community nursing, homecare, aged care, disability, rehabilitation, care coordination, or a related healthcare environment.
- Demonstrated experience conducting comprehensive clinical assessments.
- Demonstrated experience developing, implementing, monitoring, and reviewing individualised care plans.
- Experience working collaboratively with multidisciplinary teams.
- Experience coordinating services across clients, families, healthcare professionals, allied health providers, and service organisations.
- Understanding of Australian aged-care and/or disability funding and service-delivery frameworks, including Support at Home and NDIS.
- Experience identifying, managing, documenting, and escalating clinical and client-safety risks is highly desirable.

Pay: From $44.50 per hour Expected hours: 25.0 – 38.0 per week
