Clinical Care Coordinator - Office-Based (Camberwell)
Are you an EEN or Div 1 with solid clinical skills, a big heart, and quick fingers on a keyboard? We’re looking for a switched-on, compassionate nurse to join our Camberwell office team.

About the Role The Clinical Care Coordinator is responsible for overseeing the delivery of high-quality clinical care within the community setting.
You will be the go-to-support for the nurses and carers in the field, ensuring they have the support, guidance, and clinical expertise needed to deliver excellent care, a broad range of clinical skills is required for this position.
Community nursing experience is a requirement for this position.
Key Responsibilities:
- Provide clinical oversight and guidance to nurses and carers working in the community.
- Assess client needs and collaborate with families and healthcare professionals.
- Develop clients Care plans liaising with relevant services and individuals
- Managing acute nursing care from clients transitioning from hospital care to in-home care
- Managing clinical case escalations
- Maintain accurate client records and ensure compliance with care standards.
- Lead quality improvements and service enhancements.

What We’re Looking For:
- Current AHPRA registration (Registered Nurse or Endorsed Enrolled Nurse)
- Minimum 3 years’ experience in a community-based clinical role
- Strong clinical skills and experience in case management
- Ability to prioritise and respond to complex care needs
- Strong communication, negotiation, and conflict resolution skills
Required Certifications:
- Current CPR and Manual Handling Certification
- COVID-19 and Influenza vaccinations
- NDIS Screening Check, Working with Children Check, and Police Check
