About the role

We are seeking experienced and organised Community Registered Nurses with a minimum of three years' post-registration nursing experience who can provide safe, person-centred and evidence-based care in clients' homes and community settings. You will independently manage an allocated and varied routine caseload, apply well-developed clinical judgement, provide appropriate guidance to less experienced staff within scope, and promptly escalate complex, unstable or unfamiliar needs.

Regular work-related travel is an inherent requirement of the role, primarily throughout Wollongong, Wollondilly, Wingecarribee and Goulburn, with occasional travel to Campbelltown. Nurses must use their own reliable, registered and roadworthy vehicle when travelling to clients' homes and community appointments and must hold motor vehicle insurance that permits work-related travel.

Key responsibilities

- Deliver focused and comprehensive nursing assessments, including health history, observations, risk identification, care priorities and recommendations
- Conduct skin and wound assessment, cleansing, measurement, clinical photography with consent, evidence-based dressing selection, monitoring and escalation of deterioration, infection, pain, bleeding, pressure injury or delayed healing
- Perform urinary catheter assessment, routine catheter and drainage-system care, troubleshooting, client education, and clinically authorised insertion or replacement of indwelling urethral catheters
- Manage changes of established suprapubic catheters within documented education, competency and prescribed catheter plans, with immediate escalation of complications
- Administer prescribed intravenous antibiotics under an approved medication order and home-infusion plan, where credentialled and assessed as competent
- Maintain accurate, contemporaneous clinical notes, medication records, wound and catheter charts, assessments, incident reports and care-plan updates
- Provide client and family education about wound protection, catheter care, infection prevention, symptom recognition and when to seek assistance
- Collaborate with clients, representatives, prescribers, GPs, pharmacists, allied health professionals, hospitals and support teams to promote continuity of care
- Participate in documentation review, incident follow-up,



clinical audit, practice improvement and routine guidance for less experienced staff where appropriate

About you

- Minimum of three years' post-registration nursing experience, including at least 12 months of recent and directly relevant experience in community, home-based, disability, aged care, primary care or hospital-in-the-home services
- Demonstrated experience independently managing a varied community nursing caseload, prioritising competing clinical needs, coordinating care, applying sound clinical judgement and obtaining timely support for complex or deteriorating presentations
- Highly developed competence in focused and comprehensive nursing assessment, care planning, clinical risk identification, recognition and management of deterioration, evaluation of outcomes, referral and prompt escalation
- Demonstrated wound-care experience, including wound and skin assessment, measurement, aseptic technique, evidence-based dressing selection, pressure-injury prevention, infection recognition, pain assessment and clinical documentation
- Documented education and current competence in adult indwelling urethral catheter insertion or replacement and changing established adult suprapubic catheters relevant to the allocated caseload
- Medication-management competence; where IV antibiotics may be assigned, demonstrated recent competence in IV medication administration, vascular-access assessment, aseptic non-touch technique, adverse-reaction monitoring, anaphylaxis response and emergency escalation
- Strong digital capability and accurate, timely use of electronic clinical records, medication records, wound and catheter documentation, mobile communication systems and secure clinical photography
- Excellent communication and interpersonal skills with demonstrated ability to communicate clearly, respectfully and professionally in writing and verbally with clients, families, carers,



colleagues and health professionals
- Strong time management and organisational skills with demonstrated ability to plan and prioritise visits, manage a varied caseload and competing clinical demands, and complete documentation within required timeframes
- Current unrestricted general registration as a Registered Nurse with the Nursing and Midwifery Board of Australia, with no conditions, undertakings or restrictions incompatible with the role
- Current unrestricted driver licence and regular access to your own reliable, registered and roadworthy vehicle, with motor vehicle insurance that permits work-related travel
- Current nationally recognised cardiopulmonary resuscitation certification and first aid qualification appropriate to the role
- Current NDIS Worker Screening Clearance for risk-assessed NDIS duties and an acceptable aged-care screening check or clearance applicable to the service setting
- Current Working with Children Check where the role includes contact with children
- Completion of the NDIS Worker Orientation Module and all required organisational induction and mandatory training, including infection prevention, manual handling, medication safety, privacy, incident management, safeguarding, lone-worker and emergency procedures
- Successful completion of organisational competency assessments for wound care, catheter care and any other high-risk clinical duties allocated to the role
