What Your Day Could Look Like As Coordinator Home Care, you'll play an important role in helping older people maintain their independence and wellbeing while remaining connected to their community. Every Day, You'll Contribute By
- Coordinating person-centred home care services that support client goals, wellbeing and independence.
- Building strong partnerships with clients, families, health professionals and community organisations to deliver seamless care.
- Leading and supporting a team to provide safe, responsive and high-quality services.
- Identifying opportunities to improve service delivery, quality outcomes and client experiences.

You bring compassion, integrity and respect to every interaction, while confidently coordinating services and supporting teams to deliver meaningful outcomes. You'll take pride in helping older people live well at home and creating positive experiences for clients, families and colleagues alike. Here's What Will Help You Thrive In This Role
- Experience coordinating community, aged care or home-based support services.
- Strong communication and relationship-building skills across diverse stakeholders.
- Demonstrated ability to lead, support and develop team members.
- Current C class driver's licence and willingness to travel as required.
You'll stand out from the crowd if you have a qualification in individual support,
ageing or a related field and/or experience working in rural or regional communities. Above all, you'll bring Compassion, Integrity, Dignity, Innovation and Courage as you support people to live their best lives at home.
