About the role The primary role is to manage a pool of Future Butterfly clients who receive disability services. You will visit the people we support in their homes and work with them, their families, guardians and representatives to develop the plans and assessments that shape their supports, in line with the NDIS Practice Standards. Secondly, you will cultivate relationships and use existing sector relationships to build Future Butterfly's referral pipeline through face-to-face meetings with support coordinators, allied health and plan managers.
Key responsibilities
- Conduct home visits with participants across Melbourne metro
- Develop, review and document NDIS participant support plans, service agreements and risk assessments to Practice Standards
- Identify, manage, document and escalate risk, including regulated restrictive practices
- Maintain participant documentation to a standard that withstands audit
- Contribute to improving internal templates, processes and risk assessment tools
- Build and maintain referral relationships across the disability sector
- Represent Future Butterfly to participants, families and the wider sector
About you
- Substantial recent experience in disability service delivery, ideally client management with a care provider, or in support coordination
- Demonstrated capability in risk identification, management, documentation and escalation
- Working knowledge of regulated restrictive practices and provider reporting obligations
- Experience developing NDIS plans, assessments or equivalent participant-facing documentation
- Current NDIS Worker Screening Check clearance already held
- Current Australian driver licence and an insured vehicle
- Valid work rights covering the hours and duration of the role
- Existing connections in the disability sector
