About the role
Victorian Transition Centre (VTC), a Premier Supported Living Group initiative, is seeking an experienced General Manager to lead the establishment, opening and ongoing operation of a specialist hospital-to-community transition service in Preston.
VTC is being developed for people who are medically ready to leave hospital but require additional time, practical support or arrangements before moving to their next community setting. The service will provide time-limited accommodation, daily-living support, transition coordination and connections with hospitals, families and community providers.
The General Manager will translate the VTC service model into practical operating systems, workforce arrangements, referral pathways and day-to-day service delivery. The role will work closely with the Board, executive leadership team and relevant professional advisers to prepare the service for opening and lead its ongoing performance.
This is a hands-on leadership role requiring sound operational judgement, strong people leadership and experience in health, aged care, transition care, subacute services, community health or a related workplace.
Key responsibilities
Commissioning and operations
- Lead the commissioning and opening plan, including site readiness, equipment, suppliers, systems, workforce and first admissions.
- Establish safe and effective operating arrangements for referrals, assessment, admission, support delivery, escalation, incidents, discharge and transition.
- Manage service quality, risk, workforce performance, budgets, purchasing and operational reporting.
- Develop constructive referral and working relationships with hospitals, clinicians, families, community providers and other relevant stakeholders.
Policies, procedures and governance
- Develop, implement and maintain the policies, procedures and standard operating procedures required for safe and compliant service delivery.
- Ensure staff understand and follow current procedures through appropriate induction, training,
supervision and review.
- Monitor incidents, complaints, risks, compliance and service performance, and lead corrective and continuous-improvement actions.
- Work with appropriately qualified clinical advisers and providers to define clinical responsibilities and service boundaries.
Registration and funding
- Identify and support the registrations, licences, approvals and notifications required for the service to operate.
- Assist with applications and ongoing requirements relating to relevant community, health or aged-care funding, grants, tenders and service agreements.
- Ensure operations remain consistent with applicable legislation, standards, funding conditions and contractual obligations.
Board accountability The General Manager will report directly to the Board of Directors and will be accountable for VTC’s commissioning, operational performance, financial management, compliance, risk management and development.
The role will provide clear reporting on progress, performance and material risks, implement Board-approved plans and budgets, and manage the service within approved delegations. The Board will retain responsibility for governance, strategic direction, approval of the business plan and budget, oversight of risk and compliance, and other matters reserved to the Board.
Leadership and support The General Manager will coordinate delivery across operational, workforce, financial, quality and service-development priorities, drawing on relevant professional and organisational support. The nature of available support will be confirmed with shortlisted candidates.
About you
You will be an experienced operational leader with a demonstrated ability to establish, improve or lead a service. You will bring:
- senior management experience in residential aged care, healthcare, transition care, subacute services, community health or a related environment;
- experience managing teams, workforce planning, budgets, service quality and operational performance;
- experience developing or implementing policies, procedures, SOPs and governance systems;
- the ability to support registrations, approvals, funding arrangements and service agreements;
- strong communication and stakeholder-management skills; and
- sound judgement, financial discipline and a person-centred approach to service delivery.
Experience with hospital discharge pathways, the Transition Care Program, short-term restorative services or supported community transitions will be highly regarded.
Relevant qualifications in nursing, allied health, health services management, business or a related discipline are welcome. Equivalent senior operational experience will also be considered.
