Primary objective

As an advanced practitioner, ensure care coordination of a culturally safe and responsive health care services through the provision of treatment of illness, promoting holistic health and wellbeing [within a remote community to individuals, families and communities.

Context statement The Aboriginal Health Practitioner classification level AHP4 [Clinical Generalist] is an advanced practitioner, is able to work autonomously, can anticipate and set long term goals for individuals and families.



Note holistic health care services encompass the physical, mental, social and emotional, environmental and spiritual health and wellbeing of Aboriginal and Torres Strait Islander families and communities. Note this position will be located at Belyuen Health Centre based in Belyuen.

Key duties and responsibilities

1. Provide culturally safe and appropriate information regarding chronic disease, its prevalence in Aboriginal and/or Torres Strait

Islander populations, and encourage prevention, early detection and early intervention to achieve better health outcomes.
- In partnership with other healthcare professionals, develop case management and healthcare plans for Aboriginal people, and support Aboriginal people in complying with their care plan.
- In collaboration with key stakeholders, plan, organise and evaluate primary health care programs to address identified needs in

Aboriginal families and/or communities to strengthen community involvement in health service delivery
- Provide supervision and support to new graduates and trainees; including maintaining skilled boundaries and confidentiality during all supervision activities; and providing feedback which reinforces strengths and sensitively addresses the potential for change.

in order to ensure high quality, safe services and workplaces.

Selection criteria

Essential

1. Current registration with Ahpra as an Aboriginal and/or Torres Strait Islander Health Practitioner.
2. Minimum AQF level 5 as per NTPS Aboriginal Health Practitioner 2025-2029 Enterprise Agreement or be able to demonstrated

sufficient sustained experience and practice to satisfy the CEO they should appointed to that level.
- Proven extensive experience employed as a registered Aboriginal and/or Torres Strait Islander Health Practitioner providing health care services within a culturally and clinically safe manner.
- Demonstrated experience providing support and wellbeing to Aboriginal health practitioners including providing feedback and contributing to practice improvements.

Demonstrated local Aboriginal knowledge with the ability to engage and effectively communicate with families and communities to plan, organise and evaluate primary health care programs.
