About The role The Activity Lead oversees and coordinates assigned healthcare projects maintaining whole-of-activity oversight to ensure work is delivered in line with relevant funding obligations, internal processes and commonwealth guidance.

The role works with internal and external stakeholders to support effective service design and delivery, and to enhance health system integration and improve care pathways within primary care. The Activity Lead operates at a systems level, collaborating with governance bodies, service providers, and stakeholders to deliver measurable outcomes aligned with WAPHA's strategic objectives.

?Employment Type: 2 Full time roles available for a 2-year maximum term contract (part time may be considered)

? Location: WAPHA Head Office, Level 2, 1 Hood Street, Subiaco (Hybrid role)

? Salary: Band 5 ($114,442.06k - $128,750.29 Plus 12% Superannuation)

Key tasks and responsibilities:

- Lead the planning, commissioning, implementation and monitoring of activities with internal and external stakeholders,



with a focus on building capability and capacity within general practice settings.
- Monitor activity implementation, identify risks and opportunities, and make adjustments to optimise outcomes.
- Strengthen service integration and system partnerships to optimise care pathways.
- Identify service gaps and opportunities to improve access, health equity, integration, patient and clinician experience, and use of health resources.
- Support the development and continuous improvement of activity service models, processes, tools and resources.
- Apply structured project methodology, processes, and tools to create relevant plans and deliver and monitor approved activities.

About you To be considered for this role, candidates must meet the following selection criteria: Essential Experience & Qualifications

- Tertiary qualification in health or human services or equivalent level of expertise.
- Demonstrated experience successfully delivering end-to-end activities using appropriate tools to plan, manage and monitor progress towards goals.
- High-level interpersonal skills with the ability to engage and negotiate with diverse stakeholders.
- Flexible and adaptable; able to work in ambiguous situations.
- Comprehensive knowledge and understanding of health service delivery within the Western Australian health, particularly primary care and general practice.
- High level of computer literacy including proficiency in Microsoft Office suite.

Highly Regarded

- Formal project qualification.
- Familiarity with health service commissioning and healthcare funding models.
- Demonstrated experience in developing and implementing care models.
- Understanding of the Australian primary care system.
