We are excited to announce a new opportunity for the role of African Seniors Health Bicultural Support Worker (Part-time 3 days per week) – Toowoomba

Position Details:

- Part time 3 days per week
- Level 4.1 Salary + 12% Super.
- Full proficiency in reading, writing, and speaking English and one or a combination of spoken proficiency in Dinka, Arabic, Kiswahili, and Amharic, alongside a strong command of diverse African language groups and regional dialects

Role Overview: Multicultural Australia Client Services provides settlement support and case management to newly arrived humanitarian entrants, people seeking asylum and unaccompanied minors. This support creates opportunities for clients to develop the skills, knowledge and confidence needed to achieve social and economic wellbeing in their new communities.

The African Seniors Health Bicultural Support Worker delivers client-focused services that supports African Australians to live at home for as long as possible through early intervention activities and models of care for chronic health management.

The role will also increase awareness in the local primary health care workforce of the needs of the local senior population, and the availability of these initiatives.

Key responsibilities

- Determine the older persons circumstances and reasons for referral and make recommendations for, or connect to primary care, at home care or community care services
- Develop support plans together with the older person to identify their needs and set agreed and measurable goals
- Support older persons to access coordinated health appointments
- Make referrals to health and allied health professionals, and community service providers




- Identify and advocate for the health and wellbeing needs of clients and communities
- Apply an understanding of health systems from a refugee and settlement perspective
- Review materials, resources and services to ensure they are accessible, culturally safe and appropriate for clients
- Complete urgent referral back to older persons general practitioner or Nurse navigator where a noticeable decline noted and or change of living circumstances
- Visit older persons in their homes and other community settings
- Maintain relationships with other health and community services

About you

- Full proficiency in reading, writing, and speaking English and one or a combination of spoken proficiency in Dinka, Arabic, Kiswahili, and Amharic, alongside a strong command of diverse African language groups and regional dialects
- Sound and accurate knowledge of Australian culture, systems and services relevant to settlement, with the ability to clearly explain essential life skills and practical information to support clients to establish independent lives in Australia
- Demonstrated ability and willingness to work effectively across cultures, supporting clients to maintain connections with their culture of origin while developing meaningful social and community connections in Australia
- Demonstrated ability to provide culturally informed advice and guidance to clients and Multicultural Australia staff




- Demonstrated commitment to working respectfully with all members of the community, regardless of cultural or religious background, and to upholding principles of social justice and human rights
- Valid Queensland Driver's Licence
- Possession of, or willingness and eligibility to obtain a Blue Card in accordance with the Working with Children Check Act 2000
- National Police Clearance Check
