Lead a passionate team delivering high-quality, person-centred care through the Australian Government's Support at Home program. This leadership role combines clinical governance, team development, operational excellence, and quality improvement to help older Australians live independently with dignity and choice.

The Team Leader Care Management provides operational leadership, clinical governance, and day-to-day supervision of Care Partners and Intake staff delivering services under the Australian Government's Support at Home Program. The role ensures participants receive safe, high-quality, person-centred care while leading staff performance, managing complex clinical matters, driving continuous quality improvement, and ensuring compliance with aged care legislation and quality standards.

YOU WILL BE RESPONSIBLE FOR:

You will provide leadership across the Support at Home Program by overseeing care management, intake, and participant services to ensure protected, coordinated, and person-centred care. The role includes supervising and mentoring Care Partners, managing complex clinical matters, monitoring participant outcomes and budgets, supporting compliance with aged care standards, collaborating with allied health and external providers, and driving continuous quality improvement and service development.

WHAT YOU NEED TO SUCCEED:

What you will need to succeed

- Tertiary qualification in Social Work, Nursing,



or Allied Health (AHPRA aligned where applicable).
- Demonstrated experience leading a Support at Home care management team.
- Strong understanding of the Support at Home Program, Aged Care Reforms, and the Aged Care Quality Framework.
- Proven ability to supervise, mentor, and support case management staff through change and complex service delivery.
- Excellent communication, negotiation, problem-solving, and stakeholder relationship skills.
- Experience managing budgets, financial records, reporting, and database systems, with strong Excel skills.
