Holstep Health in Victoria leads a Team Leader Care Management role within the Government's Support at Home program. You will oversee care management, intake and participant services to ensure secure, coordinated, and person-centred care for older Australians.

You will supervise Care Partners and Intake staff, drive quality improvement, manage complex clinical matters, and ensure compliance with aged care standards while engaging with external providers and budgeting.
