We’re seeking a compassionate and skilled Clinical Care Coordinator in Hervey Bay to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region. Working closely with a multidisciplinary team, you’ll independently support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, collaborative practice.

Responsibilities

- Conducting clinical assessments and developing tailored treatment plans
- Deliver evidence-based clinical and psychosocial interventions
- Autonomously manage caseload and conducting risk assessment with management plans
- Collect data and supporting monitoring and evaluation
- Collaborate with teams and external stakeholders
- Facilitate care planning and review processes
- Monitor and escape mental health needs as appropriate

Requirements

- Tertiary qualification in Psychology, Social work, Mental Health Nursing, or similar
- Current Mental Health Accreditation with AHPRA Registration or eligibility for AASW Membership




- Experience working with a trauma informed, recovery focused approach supporting people with severe mental health challenges
- Experience working with diverse cultures, especially First Nations people
- Strong teamwork and collaboration skills, problem-solving and organisational abilities
