Due to our expanding client-based growth we are seeking a suitable qualified Care Partner to join our Support at Home team. The primary purpose of the role is to operate a high quality, financially sustainable home care service, while driving growth and opportunity. The role encompasses supporting clients from the initial meeting and guidance through navigating the My Aged Care system, to working with clients to develop goals that promote choice, control and independence. Working within a rights-based, person-centred framework, the Care Partner collaborates with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive and inclusive support.
They play a key role in planning and overseeing care service delivery across all support streams, including the Restorative Care Pathway, End-of-Life Pathway, and Assistive Technology and Home Modifications (AT-HM), and work closely with clinically qualified Care Partners when a participant’s needs require advanced clinical input or complex care planning and local home visits.
KEY SELECTION CRITERIA
- Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience
- EN’s with community experience highly desirable
- Current Class C Drivers Licence with reliable suitable vehicle to transport clients.
- COVID vaccinations evidence x 2 mandatory for PCL
- A current National Police Check/NDIS Workers Screening or willingness to obtain one.
- Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services
- Solid understanding of the Support at Home Program, Quality Standards, and aged care rights
- Understanding of and experience in working with quality improvements systems in an Aged/Community Care or health setting
- Excellent IT, computer, and database skills, including staff rostering software.
- Demonstrated high level communication and interpersonal skills, including report writing.
- Demonstrated high level time management and problem-solving skills and the ability to prioritise workload.
- Familiarity with My Aged Care portals and digital case management systems
