As part of our growing Support at Home service, you'll work within a contemporary, clinically-led model that places prevention, early intervention and reablement at the centre of care. Rather than waiting for health challenges to arise, we partner with consumers to proactively maintain their wellbeing, independence, mobility and confidence, enabling them to continue doing the things that matter most.

As a Care Partner, you'll build meaningful relationships with consumers and their families, supporting them to achieve their goals and maximise their quality of life. Working alongside allied health professionals, nurses and Amplar Health's broader primary care network, you'll contribute to a coordinated and personalised approach that empowers consumers to remain connected, capable and thriving in their own homes for longer.

About the Role:

As an Enrolled Nurse Care Partner, you'll work closely with consumers, families and healthcare providers to deliver person-centred care and support that promotes independence,



wellbeing and healthy ageing. Supported by a Senior Care Partner (RN), you'll build trusted relationships with consumers, undertaking holistic assessments, developing tailored care plans and coordinating services that align to each person's goals and aspirations.

Working through a preventative and reablement lens, you'll help consumers maintain and improve their functional capacity, confidence and quality of life. You'll monitor wellbeing, identify opportunities for early intervention and empower consumers to take an active role in achieving their health and lifestyle goals.

As part of a multidisciplinary team, you'll collaborate with allied health professionals, nurses and Amplar Health's broader primary care network to deliver coordinated, connected care that enables consumers to remain independent, engaged and thriving in their own homes for longer.

About You:

You are an experienced Enrolled Nurse (AHPRA registered) with:

- At least 2 years' experience in aged care and community or home care
- Strong assessment, care planning and care coordination skills
- A passion for person-centred care and building trusted relationships
- Confidence using digital systems and virtual care tools
- Excellent communication and clinical judgement
