About the role
As Case Manager for Support at Home, you will be responsible for the client journey from initial enquiry and assessment through to ongoing care coordination and review. This role combines case management with community engagement and business development. You will manage a portfolio of clients, meet with families, complete assessments and care plans, coordinate services, and help grow the Support at Home client base by building relationships with referral partners and health professionals.
Key responsibilities
- Managing a portfolio of Support at Home clients
- Meeting with new clients and their families
- Completing client assessments, care plans and regular reviews
- Coordinating services based on individual client needs and goals
- Monitoring client budgets, funding and service utilisation
- Maintaining regular communication with clients and families
- Identifying changes in client needs and coordinating additional support
- Working closely with support workers and other members of the team
- Liaising with GPs, allied health professionals, hospitals and other service providers
- Building relationships with local referral partners and health professionals to grow the client base
About you
- Experience in aged care, home care, Support at Home, community care or case management
- Understanding of the needs of older Australians and their families
- Experience developing and reviewing care plans
- Confident managing client services and coordinating multiple providers
- Excellent communication and relationship-building skills
- Highly organised and reliable with follow-up
- Confident meeting new people and developing professional relationships
- Comfortable getting out into the community
- Strong written and computer skills
- Current Australian driver's licence and reliable vehicle
