Rise is looking for a Case Worker to support young people across the City of Swan and Shire of Mundaring through outreach support, case management and youth engagement programs. This is a hands-on role where you'll manage a caseload of young people while also leading activities and programs at the Stratton Edge Youth Centre.

What You'll Be Doing

- Providing case management support to young people experiencing a range of challenges
- Developing, implementing and reviewing support plans
- Managing an outreach caseload across the City of Swan and Shire of Mundaring
- Acting as the lead worker at Stratton Edge Youth Centre
- Facilitating drop-in sessions, youth activities and school holiday programs
- Building and maintaining relationships with schools, community organisations and other support services
- Coordinating referrals and linking young people with appropriate supports
- Maintaining accurate case notes and records within Rise's client management system
- Preparing reports for funding partners including the Department of Communities and City of Swan

- A qualification in Community Services (or similar),



currently working towards one, or relevant experience in a similar role
- Experience supporting young people experiencing complex challenges
- A strong understanding of the issues and barriers impacting young people at risk
- Well-developed communication and relationship-building skills, with the ability to engage effectively with young people, families and community stakeholders
- Experience coordinating youth activities, events or group programs
- The ability to work independently and confidently within a community setting
- Strong organisational skills and the ability to manage competing priorities
- IT skills, including using Microsoft Office and client management systems
- Strong record-keeping and report-writing skills, with attention to detail
