Make a difference through advanced social work practice and clinical leadership as part of the Assertive Case Management Team within the Drug and Alcohol Service, supporting patient-centred care across the Central Coast.

About The Opportunity

Join Central Coast Local Health District as a Social Worker Level 3 and use your advanced clinical expertise to provide high-quality, evidence-informed care to patients, families and carers. Working as part of a multidisciplinary team, you will contribute specialist knowledge, clinical leadership and service improvement to support positive outcomes for the people we serve. In this role you will:

- Provide advanced social work assessment, intervention and care planning for complex presentations, using independent professional judgement and evidence-informed practice.
- Provide specialist clinical leadership, mentoring and supervision to less experienced social workers and students.
- Collaborate with patients, families, carers, multidisciplinary teams and external service providers to support coordinated, patient-centred care.
- Provide specialist advice, education and support to enhance clinical practice and build the capability of colleagues and the broader health service.
- Identify and lead opportunities for continuous improvement, contributing to innovative and effective models of care and improved service outcomes.

About You

You are an experienced Social Worker with robust clinical expertise and the confidence to work independently with complex presentations. You bring a collaborative, patient-centred approach and are passionate about supporting colleagues, improving practice and delivering high-quality outcomes. We are looking for someone who has:





- Significant social work experience within a recognised clinical specialty and a strong understanding of the psychosocial impacts of acute and chronic health conditions.
- Advanced clinical reasoning, problem-solving and decision-making skills, with the ability to manage complex cases with minimal supervision.
- Excellent communication, negotiation, advocacy and interpersonal skills, with the ability to build effective relationships with a diverse range of stakeholders.
- Demonstrated experience providing clinical supervision, mentoring, education and guidance to support the development of colleagues and students.
- Strong organisational and time management skills, with the ability to prioritise a complex workload while maintaining professional and ethical standards.
