About the Role Carbal is looking for an organised and people-focused CHSP Care Coordinator to support the day-to-day coordination of services for clients accessing our Commonwealth Home Support Program.

This is a varied role combining client contact, administration, scheduling and service coordination.

You'll be involved from the time a client first connects with the service — supporting enquiries, intake and onboarding, coordinating services and rosters, maintaining accurate client information and helping ensure services continue to meet each client's needs.

You'll also undertake scheduled client reviews and work closely with the broader CHSP team to make sure services are organised, responsive and running smoothly.

What You'll Be Doing Client Intake & Coordination

- Respond to client enquiries and communications
- Coordinate client intake, onboarding and admissions
- Complete intake documentation and ensure client information is accurate
- Help clients understand and access available CHSP services
- Maintain accurate and up-to-date client records in Visual Care

Service Coordination & Rostering

- Coordinate and schedule CHSP services
- Maintain and develop client rosters to support reliable service delivery
- Work with the CHSP team to respond to changes in client needs or service arrangements
- Coordinate weekly Carbal Kitchen client orders and associated administration
- Maintain service data and client information systems

Client Reviews & Ongoing Support

- Conduct scheduled 12-month client reviews
- Maintain current and compliant client files and service records
- Identify changes in client circumstances or support requirements
- Assist with service adjustments where required
- Help ensure clients continue to receive services appropriate to their needs

Essential

- Have experience in administration, aged care, community services, health or a similar environment
- Are highly organised and comfortable managing competing priorities
- Have excellent communication and interpersonal skills
- Enjoy working with clients and building positive relationships
- Can work independently while also being a strong team member
- Are confident using computer systems and client management software, or are willing to learn systems such as Visual Care




- Have strong attention to detail and understand the importance of accurate client records
- Are comfortable coordinating schedules and responding when arrangements change
- Are committed to culturally safe, respectful and person-centred service
- Hold a current C Class Driver Licence
- Hold, or are able to obtain, a Blue Card and NDIS Worker Screening Clearance

Desirable

- Previous experience in aged care, CHSP or community-based services
- Understanding of My Aged Care
- Experience coordinating client services, appointments or rosters
- Experience using Visual Care or a similar client management system
- Experience supporting Aboriginal and Torres Strait Islander clients and communities
- Certificate III or IV in Community Services, Aged Care or a related field
