BRIEF The CHSP Officer provides administrative, coordination and operational support to the CHSP Manager and CHSP Team Leaders in the delivery of Holdsworth’s Commonwealth Home Support Program (CHSP) services. The role supports all CHSP service streams, including Transport, Meals, Individual Social Support, Group Social Support, Domestic Assistance, Allied Health and Sector Support & Development.
Reporting to the CHSP Manager and working closely with CHSP Team Leaders, this role helps ensure day-to-day CHSP operations run smoothly, that client and program records (including DEX reporting) are accurate and up to date, and that clients, carers, staff and volunteers receive timely and respectful support.
RESPONSIBILITIES & DUTIES
- Provide day-to-day administrative and operational support to the CHSP Manager and CHSP Team Leaders across all CHSP service streams.
- Assist with the coordination of service delivery activities, including scheduling and rostering support, and the maintenance of service delivery records.
- Maintain accurate and up-to-date client, service delivery and program records, including timely and accurate data entry into DEX and other reporting systems.
- Respond to routine client, carer and community enquiries, escalating more complex matters to the relevant CHSP Team Leader or CHSP Manager.
- Support the onboarding, training and administrative needs of support workers, drivers and volunteers.
- Assist in the preparation of reports, correspondence, rosters and other documentation required for CHSP compliance and funding obligations.
- Help monitor service output data (e.g. trips delivered, meals provided, group sessions held) and flag discrepancies to the CHSP Manager or relevant Team Leader.
- Support the implementation of wellness and reablement principles in everyday administrative and coordination tasks.
- Assist with the coordination of internal audits,
compliance reviews and quality checks.
- Provide general office and administrative support to the CHSP team, including filing, data management and diary/calendar coordination.
- Support the organisation of team meetings, training sessions and other CHSP team activities.
- Contribute to continuous improvement by identifying opportunities to streamline administrative and operational processes.
- Assist with tasks and projects relating to the organisation’s transition to the Support at Home program.
- Maintain confidentiality and adhere to Holdsworth’s policies and procedure requirements at all times.
- Participate in the Holdsworth After Hours Service on a rotating basis, providing telephone support to clients and Support Workers outside standard business hours.
- Undertake other duties as delegated by the CHSP Manager and above.
EXPERIENCE
1 – 3 years’ experience in an administrative, coordinator or customer service role within aged care, community services or a CHSP-funded program (highly desirable).
QUALIFICATIONS & CERTIFICATIONS
Certificate III or IV in Business Administration, Aged Care, Community Services or similar (desirable)
- Current Driver’s Licence
- First Aid and CPR certificate
- Police Check clearance
- Working with Children Check (if applicable)
PERFORMANCE INDICATORS
- 100% of allocated administrative and reporting tasks completed accurately and within agreed timeframes
- 100% completion of allocated DEX data entry within deadlines
- Client, carer and stakeholder enquiries acknowledged within policy timeframes
- Positive feedback from the CHSP Manager and CHSP Team Leaders on responsiveness and quality of support
- 100% of client and service delivery records maintained accurately and kept up to date
- Zero preventable WHS incidents
- Attend 100% of mandatory team meetings, supervision sessions and mandatory training
- Contribute to successful compliance outcomes in internal audits and quality reviews
