Here's What Will Help You Thrive In This Role

- Current registration as a Registered Nurse and a valid driver's licence.
- Strong assessment, care coordination and case management skills.
- Experience working within multidisciplinary and community-based teams.
- Excellent communication skills and a commitment to person-centred care.
