We are currently seeking a Registered Nurse to join our District Nursing team. This is a part time position/s of up to 32 hours per week.

East Grampians Health Service is based in Ararat and Willaura (Regional Victoria) and takes its name from the magnificent mountain range that provides a backdrop to both townships. We are committed to providing quality care and services that meet the needs of the community, whilst continuously striving for improvement and aiming to lead the way in provision of health related services: Community Nursing is a part of the East Grampians Health Service Community Services Department.
The Community
Nursing team is based in Ararat and provides services across the catchment area of Ararat Rural City.
Community
Nursing includes District Nursing, Palliative Care, Hospital in the Home, Hospital Admission Risk Program (HARP), Post Acute Care, Breast Care and Home and Community Care Program for younger People (HACC PYP). Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.

About The Role
District Nurses are independent practitioners who partner with consumers to create a person-centred design of care by including the consumer in a shared decision-making process. Responsibilities include assessment and care planning, facilitating and coordinating evidence-based interventions relevant to the client’s condition(s), liaising with the team and other services, and recognising and escalating patient deterioration.
Tasks include
- Wound care
- Medication management
- Catheter care
- Hospital in the home
- End of life care
About You
- Current registration with AHPRA as a Registered Nurse
- Recent Community Nursing experience
- A demonstrated commitment to providing care that is client centered, strengths based and underpinned by a re-enablement and restorative philosophy
- A current and valid Victorian driver’s license
- Ability to contribute and practice collaboratively as part of a multidisciplinary team
