Our Enrolled Nurse support our participants with social, community & recreational participation, in-home support & daily living activities, specialised transport & domestic support.

Position Overview The Enrolled Nurse is responsible for providing safe, person-centered and responsive nursing care to participants receiving services through Tender Loving Care (TLC).

The role supports participants to maintain and improve their health, wellbeing, independence and quality of life by providing nursing care appropriate to their individual needs and within the Enrolled Nurse's scope of practice.

The Enrolled Nurse works collaboratively with participants, families and carers, Registered Nurses, Support Workers, Allied Health Professionals, medical practitioners and other stakeholders to ensure appropriate health supports are coordinated and delivered.

The role is also responsible for maintaining accurate clinical documentation, supporting medication management, identifying and escalating health and safety concerns, contributing to risk management, and ensuring nursing practice is consistent with TLC policies, relevant professional standards and NDIS requirements.

Key Responsibilities

1. Participant-Focused Nursing Practice

- Provide respectful, person-centered nursing care that promotes participant choice, dignity, privacy, independence and quality of life.
- Support participants to exercise choice and control over their health and wellbeing wherever practicable.
- Develop positive and professional relationships with participants and their families, carers, guardians and advocates.
- Take into consideration each participant's individual needs, preferences, culture, communication style and personal goals when providing care.
- Support participants with complex health and disability-related needs within the Enrolled Nurse's scope of practice.
- Promote health, wellbeing and independence through appropriate education and support.
- Recognise changes in a participant's health status and promptly escalate concerns to the Registered Nurse, medical practitioner or other appropriate health professional.
- Maintain professional boundaries and respect participant confidentiality at all times.

2. Nursing and Clinical Care

- Provide nursing care in accordance with the participant's individual care plan and documented clinical requirements.
- Conduct appropriate health observations and assessments within the Enrolled Nurse's scope of practice.
- Monitor and report changes in physical, emotional and behavioural health.
- Assist with wound care, continence care, nutrition and hydration support, pressure injury prevention and other nursing interventions as required.
- Support participants with chronic health conditions and complex health needs in accordance with established care plans.
- Assist Registered Nurses and other health professionals with the implementation and review of clinical care plans.
- Follow appropriate clinical procedures, infection prevention and control requirements and workplace health and safety practices.
- Provide health-related support and education to participants, families and support staff where appropriate and within scope.
- Escalate clinical concerns and changes in participant condition in a timely and appropriate manner.

3. Medication Management and Health Monitoring

- Support medication administration and management in accordance with legislation, organisational policies, participant medication orders and scope of practice.
- Ensure medications are administered, recorded and stored appropriately.
- Maintain accurate medication administration records and report medication incidents, omissions or concerns immediately.
- Monitor participants for potential adverse reactions or changes following medication administration.




- Support medication reviews and communicate relevant observations to the Registered Nurse, medical practitioner or pharmacist.
- Ensure medication-related documentation is complete, accurate and maintained in accordance with TLC requirements.
- Participate in the development and review of health management plans and medication-related procedures where required.
- Escalate medication concerns or clinical deterioration promptly to the appropriate clinical professional.

4. Participant Safety, Risk and Safeguarding

- Identify and report actual or potential health, safety and clinical risks affecting participants.
- Participate in risk assessments and the development and review of health-related risk management strategies.
- Complete incident reports and clinical documentation accurately and within required timeframes.
- Escalate incidents, injuries, health deterioration, medication errors, suspected abuse, neglect or other safeguarding concerns in accordance with TLC procedures.
- Support compliance with mandatory reporting requirements.
- Follow emergency procedures and respond appropriately to medical or clinical emergencies within the scope of the role.
- Promote safe environments and practices for participants, employees and visitors.
- Contribute to prevention strategies relating to falls, pressure injuries, infection, medication errors and other identified health risks.

5. Communication and Stakeholder Engagement

- Communicate effectively and respectfully with participants, families, carers, guardians and advocates.
- Work collaboratively with Registered Nurses, Support Workers, Support Coordinators, Allied Health Professionals, GPs and other health professionals.
- Provide relevant clinical information to appropriate stakeholders while maintaining confidentiality and privacy.
- Participate in case conferences, clinical meetings and care planning meetings where required.
- Support effective communication between the participant's health and support networks.
- Provide clear handovers and ensure important clinical information is communicated to relevant team members.
- Respond professionally to participant and family feedback and escalate complaints or concerns appropriately.

6. Documentation and Administration

- Maintain accurate, timely and comprehensive clinical records and progress notes.
- Ensure participant health information, care plans, medication records, risk assessments and other clinical documentation are appropriately maintained.
- Update relevant systems, including MyP, Care Vision or other systems used by TLC.
- Ensure documentation reflects the care provided, participant outcomes, observations and relevant clinical concerns.
- Maintain confidentiality of participant and organisational information.
- Complete required reports, forms and clinical records within established timeframes.
- Participate in audits and provide documentation as required for compliance and quality reviews.
- Ensure all documentation complies with TLC policies, procedures and applicable professional and regulatory requirements.

7. Quality, Compliance and Continuous Improvement

- Ensure nursing practice is consistent with TLC policies, procedures and relevant NDIS Practice Standards.
- Practice within the Enrolled Nurse's qualified scope of practice and applicable regulatory requirements.
- Maintain knowledge of relevant nursing standards, legislation, policies and clinical guidelines.
- Participate in quality improvement initiatives and service reviews.
- Identify opportunities to improve participant health outcomes and the quality and safety of clinical services.
- Participate in internal audits, clinical reviews and compliance activities as required.
- Report clinical,



operational and compliance concerns to the appropriate manager or Registered Nurse.
- Contribute to a culture of continuous improvement, accountability and participant safety.

8. Teamwork and Professional Practice

- Work collaboratively as part of the TLC multidisciplinary team.
- Support Support Workers and other team members by providing appropriate health-related guidance within the Enrolled Nurse's scope of practice.
- Participate in team meetings, training and professional development activities.
- Maintain professional conduct and uphold TLC's mission, values and standards.
- Maintain current professional registration and comply with all requirements of the Nursing and Midwifery Board of Australia and Ahpra.
- Maintain professional competence through ongoing education, training and development.
- Promote a positive, respectful and accountable workplace culture.
- Perform other reasonable duties within the scope of the position and professional qualifications as directed by management.

Essential Criteria

- Current registration as an Enrolled Nurse with the Nursing and Midwifery Board of Australia (NMBA) through Ahpra.
- Demonstrated experience providing nursing care to people with disability, complex health needs and/or other support needs.
- Understanding of person-centred care and participant choice and control.
- Understanding of the NDIS environment and the needs of people receiving disability supports.
- Demonstrated ability to assess, monitor and report changes in participant health status.
- Demonstrated understanding of medication management and safe medication practices.
- Strong clinical documentation and record-keeping skills.
- Strong communication, interpersonal and stakeholder engagement skills.
- Ability to work effectively as part of a multidisciplinary team.
- Ability to identify and appropriately escalate clinical and safeguarding risks.
- Demonstrated understanding of infection prevention and control principles.
- Ability to maintain confidentiality and professional boundaries.
- Competency in Microsoft Office and relevant clinical/CRM systems.
- Current Driver's Licence.
- Current First Aid and CPR Certification.
- NDIS Worker Screening Check.
- Working with Children Check, where applicable.
- Ability to meet all mandatory training and compliance requirements of TLC.

Desirable Criteria

- Previous experience working in an NDIS, disability, community care or supported accommodation workplace.
- Experience supporting people with intellectual disability and/or complex physical or health needs.
- Experience working with participants with complex medication or health management requirements.
- Experience working collaboratively with Allied Health Professionals and other healthcare providers.
- Experience with clinical care planning and health management plans.
- Knowledge of NDIS Practice Standards and quality and safeguarding requirements.
- Experience with clinical auditing, incident management or continuous improvement.
- Additional training or qualifications relevant to disability, complex care, wound management, medication management or other areas of clinical practice.
- Experience providing health education or mentoring to support staff.
