Description

Are you passionate about improving the health and wellbeing of people in our community through social connection? Amity Health is seeking a motivated Link Worker to join our team and support participants to access community groups, programs, and services that enhance social engagement and quality of life.

About the Role

As a Link Worker, you will play a vital role in supporting participants to identify their goals and social needs, and connecting them with appropriate services, programs, and networks. Your responsibilities will include:

- Using motivational interviewing techniques to understand participant goals.
- Building and maintaining knowledge of referral pathways and community resources.
- Keeping accurate records and maintaining service mapping databases.
- Fostering collaborative relationships with stakeholders and the wider community.
- Promoting the role of social prescribing within the region.
- Contributing to program objectives through timely completion of tasks, supporting the organisational team, and ensuring program compliance and reporting.

This is a fixed-term position of up to full-time hours, based in Albany, WA, with the contract term concluding on 31 December 2027.

About You

We are looking for someone who brings experience, compassion, and strong relationship-building skills.



You will ideally have:

- Qualifications and/or experience in health, human services, or a related field.
- Experience working effectively with vulnerable or marginalised groups (e.g., migrant or Aboriginal communities).
- Proven ability to provide client-focused care.
- A genuine commitment to equitable access to health and wellbeing supports, with a person-centred approach and the ability to listen deeply to understand what matters most to each participant.
- Strong data entry and portal navigation skills.
- Excellent community engagement and relationship-building abilities.

Mandatory Requirements:

- Ability to work in Australia.
- Current ‘C’ Class Driver’s Licence.
- Current National Police Clearance.
