Western District Health Service is seeking Expressions of Interest from current employees interested in the role of Cancer Care Nurse within our Cancer Services team.
n
This advanced practice nursing role provides a unique opportunity to support patients and their families throughout every stage of the cancer care journey. Acting as a consistent point of contact, you will help coordinate care across diagnostic, treatment and follow-up pathways, ensuring patients receive timely, connected and person-centred care.
n
Working closely with oncologists, multidisciplinary teams, tertiary referral centres and external providers, you will play a key role in improving patient experiences and outcomes for people affected by cancer across our region.
n
About the Role
n
As the Cancer Care Nurse, you will:
n
n
- Provide expert care coordination and navigation throughout the cancer care continuum.
n
- Act as a key contact for patients, families and carers.
n
- Coordinate referrals, investigations, appointments and treatment requirements.
n
- Support patients to navigate often complex healthcare systems and treatment journeys.
n
- Work collaboratively with oncologists, multidisciplinary teams and external service providers.
n
- Deliver education and support to patients and carers to promote informed decision-making.
n
- Provide clinical leadership, guidance and support to colleagues involved in cancer care.
n
- Contribute to service development, quality improvement and evidence-based practice initiatives.
n
n
What We're Looking For
n
We welcome expressions of interest from Registered Nurses who have:
n
n
- Current registration with AHPRA as a Registered Nurse.
n
- A Bachelor of Nursing qualification.
n
- Demonstrated experience in oncology nursing, cancer services or care coordination.
n
- Postgraduate qualifications in Oncology Nursing (or working towards) desirable.
n
- Strong clinical assessment, triage and care coordination skills.
n
- Experience working collaboratively within multidisciplinary teams.
n
- Excellent communication, interpersonal and relationship-building skills.
n
- Strong organisational and time management abilities.
n
- A commitment to person-centred care, quality improvement and continuous learning.
n
