Western Health is recruiting an experienced Emergency Department Care Coordinator to join the EDCC Team for a 12-month fixed-term, full time role across Sunshine, Footscray and Williamstown Hospitals. You will coordinate care for patients with complex medical, psychological and social needs and work within a dynamic, multidisciplinary service.

You will lead discharge planning, collaborate with emergency clinicians and external agencies, and support safe transitions to community services while
