Our client, Triple B International, is an Australian-owned aged care, disability care and community-services provider supporting clients across metropolitan Perth.

Due to continued business growth and increasing client service requirements, our client is seeking to appoint an additional Case Manager (Community Worker). The successful applicant will join the existing case management team and report to the Senior Case Manager.

About the Role

The Case Manager (Community Worker) will assess client needs, develop and review individual case plans, coordinate services and connect clients with appropriate community resources.

The position works closely with clients, families, carers, community organisations, healthcare professionals and other service providers to support client wellbeing, independence and community participation.

The typical job responsibilities will include, but are not limited to the following:

- Assess clients’ needs, circumstances, goals and available supports.
- Develop, implement and regularly review individual case plans.
- Coordinate referrals and access to community, health, welfare, aged care and disability services.
- Liaise with clients, families, carers, community organisations and other service providers.
- Monitor service delivery, client progress and changing support needs.
- Identify and appropriately escalade client risks, incidents and safeguarding concerns.
- Maintain accurate case notes, assessments, reports and other client records.
- Contribute to community engagement and service-improvement activities.

Applicants must meet the following minimum requirements to be considered for the role:

- Current NDIS Worker Screening Check, where required.
- Current Australian driver’s licence and access to a reliable, insured vehicle.
- Current First Aid and CPR certification.
- Lawful permission to work in Australia.




- Must have relevant experience in at least one of the NDIS, aged care, disability or multicultural community-services sectors.
