Townsville Hospital and Health Service is seeking a Health Worker (Identified) to support culturally protected cardiac care for Aboriginal and Torres Strait Islander clients within a multidisciplinary team. You will assist with health promotion, prevention and early intervention and help clients navigate the health system while strengthening health literacy.

Working across in-reach and outreach services, you will build connections with communities to support continuity of care and ensure services
