Design and deliver social and wellbeing programs that enhance the lives of older people in the community.

Resthaven Marion Community Services is seeking a Social Coordinator to join the team on a full time, 6-month fixed-term contract. In this rewarding role, you'll work in partnership with older people to plan and coordinate contemporary social and wellbeing programs that support independence, engagement and a positive lived experience. With each person's choices, strengths, capacity and goals at the centre, you'll help create programs that support older people to remain living in the community.

Key responsibilities include:

- Planning and coordinating social and wellbeing programs, including managing the program calendar, bookings and activities.
n
- Working with clients to deliver programs that enhance engagement, wellness and social connection.
n
- Collaborating with Care Partners and allied health to support holistic, person-centred services.
n
- Monitoring program participation, engagement, outcomes and expenditure.
n
- Identifying opportunities to improve program promotion, engagement and uptake.
n
- Supporting staff involved in delivering social and wellbeing programs.

- Maintaining program documentation and contributing to continuous improvement.

To be successful, you'll bring:

- Relevant experience working in aged care.
n
- Empathy and understanding of the needs of older people, their families and carers.
n
- A commitment to supporting older people to remain at home in their community.
n
- The ability to work within a holistic, person-centred model of care.
n
- Effective written and verbal communication skills.
n
- Practical problem-solving skills.
n
- Computer literacy, including Microsoft Office and electronic client care systems.
n
- A current SA driver's licence.
n

Qualifications including a Certificate IV in Leisure & Health, Diploma of Community Services (Case Management), or Diploma of Ageing Studies and Services are highly regarded.
