Your new role
As An Advanced Clinical Pharmacist Within Community And Oral Health, You Will Provide Specialised Clinical And Pharmacist Community Services - Advanced Clinicalconsultancy Services Across a Diverse Range Of Community-based Programs, Supporting Safe, Patient-centred Care And Improving Medication Management Across The Hospital-to-community Continuum.

Stakeholders, You Will
- Deliver advanced clinical pharmacy services to complex community-based clients across services including Rehab@Home, Acute Care@Home, chronic disease, oral health and specialist community services, including home and community visits.
- Provide clinical leadership, expert advice, mentoring and supervision to pharmacists and other staff, supporting the development of contemporary, evidence-based pharmacy practice and capability across Community and Oral Health.
- Lead the development and implementation of innovative clinical practices, protocols, education resources and models of care that improve medication safety, continuity of care and consumer outcomes.
About You We are looking for an experienced and forward-thinking pharmacist who brings advanced clinical expertise, strong leadership capability and a passion for delivering high-quality pharmacy services in community settings. The ideal candidate will demonstrate:
- Advanced clinical knowledge and expertise in delivering high-quality, patient-centred pharmacy care, with an understanding of medication management and service delivery across the hospital-to-community continuum.
- Highly developed communication, collaboration and organisational skills, with the ability to work effectively across multidisciplinary teams and engage with consumers, community pharmacies, GPs, residential aged care services and other stakeholders.
- Robust clinical leadership, supervision and mentoring skills, with the ability to support the development of less experienced staff and build a capable, sustainable pharmacy workforce.
