About the role:

As a Registered Nurse with St Carthage's Community Care, you will provide high quality nursing and care services that support our clients to remain living at home. Our nursing team currently operates Monday - Friday between 7am-4pm. We are seeking a Registered Nurse to work a minimum 2 days a week, preferably Monday, Wednesday or Friday.

You’ll develop genuine connections with our clients and their families and assist those clients to optimise their health and wellbeing and support their independence.

A typical day can include:

- Providing support to clients including clinical care and assessment.
- Providing support, direction and orientation to RN1’s & EN’s
- Working alongside our clients, their family and Support Planners to set individual goals that are realistic and achievable
- Conducting post hospital clinical assessment
- Conducting initial clinical Assessment for new clients entering community services as well as Annual reviews
- Conducting post client incident reviews
- Supporting clients by providing clinical support and wound care
- Attending Allied Health meetings
- Attending to clinical needs or referrals to Allied Health in conjunction with Support Planners
- Liaising with tertiary and primary health professionals

The position is based in the community, working in the Far North Coast Region of NSW. You would operate from our Goonellabah office and need to be prepared to travel across the Northern Rivers to support our clients. You will be supported by a nurturing team of fellow RN,



ENs, Support Planners, Care Partners and Lead Community Care Workers.

About you To be successful in this role, you will have:

- Registered Nursing qualifications with current AHPRA registration
- 2 years clinical care experience in the Aged Care sector or transferable skills
- A caring nature with demonstrated tolerance, patience and empathy with older people
- Robust organisational/time management skills with an ability to adapt to changing demands and to work under pressure
- Team working skills plus an ability to work independently
- A current NSW Drivers Licence
- Sound written and verbal communication skills and computer skills
- Assessment and report writing skills
