Social Support Group Facilitators deliver safe and high quality culturally appropriate group based activities to clients over 65 years. The role assists older people to feel socially included through structured, group based activities that develop, maintain or support independent living, wellbeing, and social interaction. Facilitators are responsible for the safety and quality of their own skilled practice, consistent with ITSOWEL's policies and procedures, the Commonwealth Home Support Program Manual & Guidelines, and the Aged Care Quality Standards.
Key responsibilities
- Collect client attendance records as per operational procedures and plan and deliver activities.
- Build and maintain positive working relationships with service users, participants, and carers.
- Identify clients in need of additional assistance or services, and work with families to refer clients for appropriate supports.
- Support SSG clients who require Support at Home services with information and referral to My Aged Care.
- Implement strategies to obtain feedback from clients about services provided, including customer satisfaction, meals and activity preferences.
- Develop and maintain Client Support Plans in line with ITSOWEL policy and client's needs.
- Provide guidance and operational support to SSG volunteers in line with ITSOWEL policy.
- Ensure all client paperwork is up to date in line with ITSOWEL policy and procedure.
- Comply with all WH&S; policies and procedures of ITSOWEL, including Food Safety Policy.
- Handle complaints and incidents in line with ITSOWEL policy.
About you
- Minimum Certificate 3 in Aged Care; Individual Support; Community Services or equivalent substantial experience in delivering group-based programs.
- Current First Aid Certificate.
- Understanding of and commitment to consumer directed care and person centred services.
- Demonstrated experience in working within aged care services sector.
- Knowledge and understanding of aged care legislation, codes of practice, industry standards relevant to the provision of programs and services.
- Commitment to the provision of care services in a culturally appropriate manner.
- Strong time management and ability to work independently and as part of a team.
- Experience in managing groups and developing a program of activities.
- Experience managing a team of volunteers.
- Unrestricted motor vehicle licence (essential), own motor vehicle (essential), and comprehensive car insurance.
