It provides healthcare services through digital and remote consultation channels and may engage General Practitioners, Nurse Practitioners, specialists, allied health professionals, administrative personnel, technology support providers and independent contractors.

Key responsibilities

- Conduct telehealth consultations using evidence-based assessment, triage, and management pathways.
- Develop and implement care plans for chronic conditions such as diabetes, hypertension, COPD, and mental health support.
- Prescribe and manage medications within scope, perform remote monitoring and follow-up, and liaise with GPs and specialists as required.
- Provide patient education, self-management coaching, and preventative health advice tailored to diverse populations.
- Maintain accurate clinical documentation and contribute to continuous quality improvement activities.

Who we are looking for

- Registered Nurse or Nurse Practitioner registered with the Australian Health Practitioner Regulation Agency (AHPRA) under General registration.
- Australian citizen, permanent resident, or holder of a valid work (business) visa eligible to work in Australia.
- Practice nurse experience preferred; applicants with experience levels ranging from zero to 30+ years are welcome.
- Strong clinical reasoning, excellent communication skills, and comfort with digital health platforms and electronic medical records.
- Commitment to patient-centred care, cultural safety, and collaborating with multidisciplinary teams.
