What your day could look like
As an Administration Officer - Transition Care, you'll be a key part of a team that helps people navigate important stages of their care journey. Through strong coordination, effective communication and attention to detail, you'll help ensure services run smoothly and patients, families and carers feel supported every step of the way.
Every day, you'll contribute by: * Managing referrals and information workflows that enable timely access to services. * Providing responsive administrative support that helps the team deliver coordinated, person-centred care. * Building positive relationships with patients, carers, service providers and colleagues to support connected care outcomes. * Contributing to reporting, service improvement activities and efficient team operations that enhance program performance. You're an organised and adaptable professional who enjoys making a difference through the work you do. You bring initiative, sound judgement and a collaborative approach, balancing multiple priorities while maintaining a high standard of service. You understand the value of clear communication and take pride in supporting both people and processes. You'll stand out from the crowd if you have experience within healthcare,
aged care or community services and/or have been involved in continuous improvement initiatives.
