About the role and where you will be working:

The role focus is the delivery of safe, client centred and efficient services that produce the best quality service outcomes for our clients overall health & wellbeing. This will be achieved through collaboration with Home Care Operations division to support, develop and grow Home Care Services

Your responsibilities will include but not limited to:

- Grow Home Care services as measured in number of Home Care clients and service hours delivered.

- Foster and actively contribute to a culture of continual improvement and make a lasting positive impact on the organisation, its people and the community we serve.

- Support Senior Management with the planning, management, and delivery of strategic outcomes for SCCWA's Strategic Plan and work collaboratively with employees to achieve those outcomes.

- To provide leadership and direction to ensure all clients high-quality care that meet or exceed the requirements of the Aged Care Standards.

About you: In order to be successful in this role, you will need to have at least 5 years experience in a similar role. You will also need a robust commitment to person centred care and able to work both independently and within a team.
