About the role

The Aboriginal Intensive Family Support Worker (50D) is responsible to engage and work with identified at risk Aboriginal families to prevent the need for children to enter out of home care. The role is key to assisting with the planning with District staff and others to develop community and family capacity to provide a safe environment for children and other vulnerable people. The aim is to strengthen families' capacities to provide safe care by increasing child safety through the Intensive Family Support approach, providing intensive in-home support services and case management for Aboriginal children in ways that are practical and flexible, in alignment with the Department's Case Plan with the family; and responsive to the cultural needs of the family.

Key responsibilities

- Maintain an active case load as directed by the Line Manager

- Provide one to one intervention based on the family's needs and goals through intensive therapeutic interventions

- Work in line with Trauma Informed Practice Principles and Wadjak Northsides Therapeutic Intervention Model of care

- Develop and monitor assessments and case plans for service users including case reviews and safety planning in line with the Service User Journey

- Ensure appropriate therapeutic and/or community and government support services are provided to service users either directly or by referral

- Identify, assess and respond to safety concerns of service users in line with Wadjak Northside incident management procedures

- Work within a records management service for the receipt, custody, control, preservation and retrieval of records and related service user material through maintaining timely, accurate and comprehensive records of intervention including assessment and case notes

- Participate in community forums, reference groups and projects as directed by the Line Manager





- Establish and maintain connections and networks with relevant support services and government organisations

- Participate in Multidisciplinary Stakeholder meetings as required

About you

- Experience in delivering therapeutic interventions to service users that are evidence-based

- Experience working with service users who are considered at risk or are presenting with complex issues

- Experience and knowledge in working with diverse cultures and communities

- Experience in case work practice within the community service or health sectors

- Excellent oral communication and interpersonal skills

- Good administrative, recording and case management documentation skills with experience using online platforms and Microsoft Office suit

- Demonstrated ability to be innovative, flexible and work autonomously (where required)

- Driver's License

- Ability to obtain a Department of Communities 395

- Working with Children Check (WWCC)

- National Police Clearance (obtained within the last six months)

Wadjak Northside is a partner organisation providing intensive family support services in Joondalup and Mirrabooka. The service works to strengthen families' capacities to provide secure care and prevent children from entering out of home care through trauma informed practice, person-centred and strengths-based strategies.
