Clinical Care Coordinator
An established not-for-profit residential aged care provider is seeking an experienced Registered Nurse to step into a key leadership position as Clinical Care Coordinator.

This is an exciting opportunity for a clinically strong and people-focused RN to join a small residential aged care facility and play a pivotal role in delivering safe, high-quality, resident-centred care.

Reporting directly to the Facility Manager, you will be a key member of the leadership team and will act as second in charge, supporting both the day-to-day clinical operations and broader facility management. The resident cohort includes high-care residents and people living with dementia.

Responsibilities:

- Providing clinical and operational leadership to ensure the delivery of safe, high-quality, resident-centred care
- Leading, supervising and supporting Registered Nurses, Enrolled Nurses and Personal Care Workers
- Supporting the Facility Manager with day-to-day operations
- Ensuring compliance with the Aged Care Quality Standards, Aged Care Act, SIRS, AN-ACC requirements and organisational policies
- Overseeing care planning, clinical assessments, documentation and medication management
- Monitoring and responding appropriately to clinical incidents, adverse events and complaints
- Leading clinical audits, quality improvement and continuous improvement initiatives
- Supporting staffing coordination, skill mix and workforce capability
- Providing coaching, mentoring, performance feedback and professional development to clinical and care staff
- Promoting effective communication and collaboration across the multidisciplinary team
- Contributing to a positive workplace culture focused on dignity, respect and quality of life for residents

Essential Requirements:





- Current AHPRA registration as a Registered Nurse
- Minimum 3 years' experience in a senior clinical or aged care leadership role
- Strong knowledge of the Aged Care Quality Standards, SIRS, AN-ACC and clinical governance
- Demonstrated leadership and team management experience
- Experience working with multidisciplinary teams
- Solid clinical assessment, care planning and documentation skills
- Experience in high-care residential aged care is highly desirable
- Excellent communication, problem-solving and organisational skills
- The ability to confidently manage clinical complexity and competing priorities
- A genuine commitment to delivering compassionate, culturally appropriate and resident-centred care
