We are looking for a compassionate and experienced Registered Nurse to join our Support at Home team and provide high-quality, person-centred care to older people living independently in their own homes.
About the Role
As a Registered Nurse, you will:
Conduct comprehensive nursing assessments and develop individualised care plans.
Provide clinical care including wound care, medication support and health monitoring.
Monitor clients' health and respond appropriately to changes or clinical risks.
Coordinate care with GPs, allied health professionals, families and support workers.
Provide clinical guidance and support to care staff.
Maintain accurate clinical documentation and ensure compliance with relevant aged care standards.
Promote client choice, dignity, independence and quality of life.
About You
To be successful, you will have:
Current AHPRA registration as a Registered Nurse.
Experience in aged care, community nursing, home care or a similar setting.
Robust clinical assessment,
communication and care planning skills.
Ability to work independently and as part of a multidisciplinary team.
A current driver's licence and reliable vehicle, where required.
Current police check and relevant screening/clearances.
