Job Description

Monash Health in Melbourne's south-eastern region is seeking a Grade 3 Social Worker Case Manager to support adults experiencing complex mental health challenges. This is a full time, fixed-term position until March 2027, with part time applications also welcome.

Within a cooperative multidisciplinary team, you will provide recovery-oriented, person-centred care, complete psychosocial assessments, conduct home and community outreach, and contribute to duty and crisis response alongside.
