Wesley Mission is seeking a Case Manager to support children and young people in Out of Home Care (OOHC) and Supported Independent Living (SIL).
You will work with young people, birth families and carers to ensure protected, nurturing placements and help them build independence and life skills.
This role requires a social work or related degree and at least two years' experience in OOHC, with strong communication and team collaboration.
