integratedliving Australia in Traralgon/East Gippsland is seeking a Care Partner to support older clients living at home. You'll build trusted relationships, develop personalised care plans, and coordinate services to ensure a seamless, responsive care journey.

This permanent full‐time hybrid role requires appropriate aged care qualifications, experience in assessment and care planning, and robust communication and teamwork to deliver person‐centred care for clients and families.

Rates: Day $160/hr | Night $190/hr. Please note that all rates are inclusive of superannuation, taxes, and casual loading, where applicable. Must have current AHPRA registration as a medical practitioner.
