Internal Expression of Interest - District Nurse (Long Service Leave Backfill) District Nursing Service | Western District Health Service Internal Applicants Only Fixed Term Backfill Opportunity until 8 November **** Hamilton, Victoria Classification: YU15/IB71 Support clients to remain independent and receive high-quality care within their own homes Make a difference in community healthcare Western District Health Service is seeking expressions of interest from current WDHS employees for a temporary District Nurse opportunity to backfill a period of Long Service Leave.
Working as part of the District Nursing Service, you will provide person-centred, evidence-based nursing care to clients within the community setting.
The role supports individuals to maintain health, independence and quality of life while receiving care in their own homes and community environments.
This is an excellent opportunity to develop your skills in community nursing while working within a supportive and experienced team committed to delivering high-quality care across the region.
What You'll Be Doing Community Nursing Care Deliver high-quality clinical care including wound management, medication administration, assessment and ongoing care planning for clients with acute, chronic and complex healthcare needs.
Patient-Centred Care Support patients and their families through collaborative care planning, education and health promotion activities designed to maximise independence and wellbeing.
Care Coordination Coordinate services and work closely with multidisciplinary teams to ensure continuity of care across community, hospital and aged care settings.
Autonomous Practice Work independently within the community setting,
exercising sound clinical judgement and responding appropriately to changing patient needs.
On-Call Roster Participation Participate in a seven-day rotating roster, including weekends, public holidays and an on-call roster supporting palliative care clients.
What You Bring Current AHPRA registration as a Registered Nurse Diploma or Bachelor of Nursing Minimum 2-3 years post-graduate nursing experience Demonstrated clinical assessment and care planning skills Strong communication and interpersonal skills Ability to work independently and manage a community-based caseload Commitment to person-centred care and continuous improvement Current Driver's Licence NDIS Worker Screening Check and Working with Children Check (or willingness to obtain) Rights to work in Australia About WDHS At Western District Health Service, we are committed to delivering high-quality, person-centred healthcare to communities across the Southern Grampians and Glenelg regions.
