Job Description
We are seeking a compassionate and experienced
Registered Nurse
to join our Homecare team, supporting the delivery of high-quality clinical care to clients living independently in the community.
Reporting to the
Customer Care Manager
, you will play a key role in providing clinical coordination, case management, and nursing support to Homecare Package clients.
This role is ideal for someone who enjoys a mix of clinical work, care planning, and stakeholder engagement.
Key Responsibilities
Deliver high-quality, person-centred nursing care to clients in their homes
Conduct assessments, develop and review care plans to meet changing client needs
Provide clinical guidance and respond to changes in client health status
Maintain accurate and compliant clinical documentation
Collaborate with multidisciplinary teams to ensure optimal care outcomes
Build strong relationships with clients, carers, GPs, and community stakeholders
Support continuous improvement initiatives and uphold best practice standards
Ensure compliance with Aged Care Quality Standards, WHS, and organisational policies
Participate in mandatory training and ongoing professional development
About You




You are a dedicated and proactive Registered Nurse who is passionate about supporting older people and individuals living with disability to live independently.
Strong clinical knowledge and sound judgement
Excellent communication and relationship-building skills
The ability to work autonomously and within a team
A genuine commitment to quality care and continuous improvement
Key Requirements
Bachelor of Nursing with
current, unrestricted AHPRA registration
Minimum
5 years' post-graduate experience
(aged care, community, or hospital setting)
Experience in
wound care, catheter care, assessments, and care planning
Solid understanding of aged care standards, WHS, and risk management
Current
First Aid Certificate
and
National Police Check
Valid
driver's licence
and access to a reliable vehicle
