About the Role
RN2
Join our Transition Care Programme as a Clinical Nurse and help older people move confidently from hospital to the right care, health and rehabilitation services.
Coordinate TCP referrals and client intake
Develop short term care and case plans
Link clients with community and aged care services
Support staff and maintain high clinical standards
AHP2
Join our Transition Care Program as a Transition Care Program Coordinator and help older people move safely from hospital to the right community care, health and rehabilitation services.
Coordinate TCP referrals and client intake
Assess needs and create short term care plans
Link clients with community and rehabilitation services
Manage ongoing case care after discharge




Guide staff and maintain high practice standards
