At Envigor, we've been delivering community-based care across Queensland for more than 10 years through Support at Home, Veterans' Community Nursing (DVA), and NDIS services.
We're looking for an experienced Clinical Nurse - Senior Community Nurse who is passionate about community nursing, clinical excellence, and improving the lives of people receiving care at home.
This is an ideal opportunity for a senior nurse who enjoys influencing care quality, mentoring others, and improving clinical systems and processes, without the demands of direct people management. This senior practice-improvement position enables you to mentor clinical leadership and expertise to support safe, high-quality care across our services, rather than manage a standard nursing caseload.
About the role
We are seeking a proactive and solutions-focused Clinical Nurse - Senior Community Nurse to support the delivery of safe, high-quality care across our community services. In this role, you will provide clinical leadership, support the review and improvement of clinical systems and processes, and promote independence and reablement to help participants remain safely at home. You will work closely with team members to strengthen clinical care pathways, enhance the participant experience, and support the development, implementation, and evaluation of clinical assessments and care plans.
What You will Do
- Support and mentor staff in contemporary nursing processes and practice, ensuring care is responsive, rights-based, aligned with evolving healthcare practices and responsive to healthcare reforms
- Strengthen the provision of and where required complete Clinical Assessments, Care Plans & Clinical Reviews
- Enhance clinical care bundles and pathways
- Document clinical workflows clearly and accurately, ensuring staff understand key steps, responsibilities, decision points, and risks to support protected and consistent care delivery
- Work collaboratively with multi-disciplinary team members to achieve better participant outcomes
- Monitor and support compliance with clinical pathways and quality requirements
Skills & Experience - What You Will Bring
- Bachelor of Nursing
- AHPRA Registration as a Registered Nurse (no restrictions and unlimited work entitlements)
- 7+ years post registration experience
- Experience in community, aged care, disability, or chronic disease management preferable
- National Disability Worker Screening Clearance
- Vaccinations - Current Influenza Vaccination
- Current QLD Driver’s licence, and a reliable vehicle
- Strong clinical assessment and care planning skills
- Confidence using electronic clinical systems and documentation
