Silver Chain Group is seeking a Care Partner across the Sunshine Coast and surrounds to support older Australians with government funded in-home aged care services. You will manage caseloads, assess needs and develop personalised care plans, while coordinating with clinical, scheduling and service delivery teams to ensure seamless care.
