About the Role
This isn’t your average aged care physio role.
Whether you’re experienced in aged care or have been curious to step into the sector, this role offers real diversity; blending a community-style approach within an aged care home. You’ll support clients to build strength, mobility and confidence so they can live their most independent, joyful life. No two days look the same, and you’ll have the freedom to be creative,



backed by a supportive team that keeps everything running smoothly.
You’ll deliver four structured components of the Physiotherapy service for our aged care partner in this role:

1. Wellness and Reablement Programs
Deliver individually tailored wellness and reablement programs aligned with each resident’s goals and needs. Delivered through 1:1 or in a group setting; our innovative suite of wellness and reablement programs (funded under HELF) have been pre-designed by our expert Clinical Team, it will be your role to identify potential participants, ensure uptake, and work with the on-site and Guide’s clinical and admissions team to build a positive ageing culture within your home.

2. Hydrotherapy Programs
With access to a state-of-the-art Hydrotherapy pool; you’ll be responsible for coordinating and delivering weekly group classes to residents. A truly unique offering that allows for an enhanced wellness experience for residents.

3. Assessment and Care Planning
Complete core clinical tasks such as assessments, reviews, referrals, meetings (HOD/MDT/case conferences) and staff education, as part of the multidisciplinary team on site.

4. Deliver Manual Handling Training




Our on-site training focuses on creating a practical, hands-on and engaging experience for where participants are challenged to complete tasks and demonstrate their knowledge of manual handling.

Duties:

- Assess, plan and deliver evidence-based, goal orientated, and outcome-driven therapy
- Build a vibrant wellness culture that enhances resident outcomes, and engagement.
- Deliver Wellness and Reablement Programs within HELF services
- Prescribe & document exercise programs for residents
- Build strong client relationships
- Deliver therapy though 1:1's and group programs
- Coordinate and run Hydrotherapy classes
- Document clearly and efficiently
- Collaborate with clinical & multidisciplinary teams
- Report and be accountable to HELF service KPIs

Skills & Experience

- AHPRA registration
- Police Check
- Driver’s licence + reliable car
- WWVP Check + NDIS Orientation
